Continuum of Care
CAHMCO is the fiscal agent for the Continuum of Care Committee, which is a network of agencies that assist homeless individuals and families by providing emergency, transitional and permanent housing, as well as necessary supportive services. Intake, assessment and outreach services are provided through the Provider’s Communication Network (PCN), an on-line program developed specifically to provide accessible empirical data on the homeless population, as well as the inventory of housing units and services available.
The Continuum of Care meets on the second Thursday of each month.
The Continuum of Care Coordinator represents Continuum members with the Human Service Network, the PCN Committee meetings. The Coordinator also is working with a regional group to formalize the Continuum of Care process throughout metropolitan Chicago. Not only does HUD look to the Continuum of Care Committee to work together to identify the gaps in homeless housing and services, and to determine the appropriate projects and programs for McHenry County, but also the Illinois Department of Human Services (DHS) and the Illinois Department of Commerce (DCCA) are looking to the Continuum of Care process in determining how funding should be allocated to programs and projects assisting homeless individuals and families.
Continuum of Care Committee
